There is something genuinely strange about the current push to put a health tracker on every American wrist, and it is worth pausing on before arguing about whether the push is good or bad. The enthusiasm comes from two camps that agree on almost nothing else. On one side is a populist health movement built substantially on distrust of large institutions, pharmaceutical companies, and corporate influence over medicine. On the other are health and life insurers, which are large corporations. Both want you wearing a device that continuously records what your body is doing.
In a STAT First Opinion essay, sociologists Andréa Becker and Paul Kamuf argue that this convergence should worry people, and they reach for auto insurance to explain why. Their case is worth engaging seriously, and so is the case against it. But the more interesting question sits underneath both: why do these two camps want the same thing at all, and what does the answer tell you about where this ends up?
What is actually being proposed
Start with the facts, separated from the framing. Health Secretary Robert F. Kennedy Jr. has said he envisions every American wearing a wearable within four years, arguing that devices tracking sleep, heart rate, glucose, and similar measures will help people take control of their health. A White House strategy document has proposed linking wearable data to electronic health records. In January, the Food and Drug Administration reclassified AI-enabled wearables so that they are not regulated as heavily as medical devices, provided they do not claim to diagnose or treat disease.
Meanwhile, insurers have been building the commercial version of the same idea for years. Programs such as John Hancock's Vitality and UnitedHealthcare's Wellness Rewards already offer discounts, gift cards, and points to policyholders who demonstrate favorable behaviors. So the policy push and the industry push are not hypothetical futures. They are existing programs and stated plans that happen to require the same underlying infrastructure.
The shared premise, not a shared goal
Here is why two camps this different want the same device. They do not share a goal. They share a premise: that health is primarily a product of individual behavior, and that measuring individual behavior more precisely is therefore the route to improving it.
From that common premise the two purposes diverge completely. For the health movement, the point is empowerment, giving individuals the data to take charge of their own bodies rather than depending on institutions that have failed them. For insurers, the point is pricing, sorting policyholders by the risk their behavior implies. These are genuinely different aims, held by people with genuinely different motives, and neither is dishonest about what it wants.
But they require exactly the same thing to be built: a continuous, granular, individual-level record of how a person's body behaves. And infrastructure of that kind does not stay attached to the intention that motivated it. It gets built once, and then it serves whoever ends up holding it.
Empowerment and accountability are the same data
That is the crux of the matter, and it deserves to be stated plainly. A wearable that tells you your sleep has been poor for three weeks, so that you can do something about it, is the same wearable that tells an insurer you sleep poorly. The reading does not change depending on the reason someone wants it. Empowerment and accountability are not opposite uses of data. They are the same data, pointed in different directions.
This is why the auto-insurance comparison the essay's authors draw has real force, whatever one concludes from it. Telematics programs were also introduced as tools for drivers, with the promise that safer driving would be rewarded. The authors note a Maryland Insurance Administration review that found a mixed picture of premium changes rather than clear overall savings, and they argue that the data's most reliable use has been commercial rather than protective. Whether health wearables follow that path is not yet determined. But the structural point holds regardless of how the auto case is read: once the measurement exists, its use is decided by whoever has access to it, not by whoever first argued for it.
The variable that actually decides this is legal, not technological
And that points to something neither camp in this fight tends to say out loud, which is that the technology is not the thing that determines the outcome. The law is.
Auto insurers can price you on your driving data because pricing risk that way is permitted. American health insurance works differently. In most markets, insurers are largely barred from setting individual premiums based on health status or pre-existing conditions, which is precisely the practice that would make a wearable into a pricing instrument. The significant exception is workplace wellness programs, where regulations do allow financial incentives tied to health-related activity within limits. So the danger the essay describes is not conjured by the device. It depends on how much room the rules leave for health status to affect what an individual pays.
Which means the wearables debate is, in substance, a proxy fight over rating rules and the size of the wellness-program exception. If those protections hold, a wearable is a fitness tracker with a data-privacy problem, which is serious enough on its own. If they erode, the same device becomes the mechanism for individualized health pricing. The technology is identical in both worlds. Only the legal architecture differs, and that, not the hardware, is where the question is actually settled.
When "voluntary" stops feeling voluntary
There is a related mechanism worth understanding, because it is how these programs expand without anyone mandating anything. Wellness and telematics programs are structured as opt-in: share your data, receive a discount. Nobody is compelled.
But a discount for participants is arithmetically indistinguishable from a surcharge for everyone else, and as participation becomes the norm, declining to share increasingly means paying more for the same coverage. The choice remains formally free while becoming practically expensive, which is a familiar pattern in insurance and not unique to this technology. Whether that constitutes coercion or simply pricing is a fair question on which people disagree. What is not in dispute is that opt-in framing understates how much pressure such programs eventually apply.
The evidence is thinner than either side suggests
Honesty requires noting that the empirical case is weak in both directions, and that both camps oversell. The optimistic claims outrun the evidence: the essay's authors point out that the clinical usefulness of consumer wearable data is limited, that the readings are often inaccurate, and that it is unclear whether wearables make people more health-conscious or simply appeal to people who already are. Meanwhile, evidence that these programs reduce overall costs is not strong.
But the pessimistic claims are also predictions rather than findings. Widespread weaponizing of wearable data against American policyholders has not happened, and under current rules in most markets it would face real legal obstacles. So a fair reading is that this is a plausible risk worth taking seriously and guarding against, not a documented harm, and that the honest position on both the promise and the peril is considerable uncertainty.
The strongest version of each side
Because this is contested political ground, each position deserves its best statement, and this analysis takes no side among them. The case for the push is real: chronic disease is a genuine and serious burden, people have a legitimate claim to their own health data, prevention is generally cheaper and kinder than treatment, and there is something defensible about a health movement wanting individuals to hold information about their own bodies rather than leaving it solely with institutions. Insurers can fairly say that their programs are voluntary, that rewarding healthy behavior is not sinister, and that participants who value the discounts are capable of deciding for themselves.
The case for caution is also real. Privacy risks around continuous bodily data flowing to corporations are concrete, including questions about storage, security, and sale to third parties that the authors correctly say remain unanswered. And their broader structural objection deserves weight: a framework centered on individual optimization can obscure how much of health is shaped by income, environment, occupation, and access to care, none of which a wristband addresses. Nobody's step count prevents a cancer diagnosis or a drunk driver, and a policy conversation that drifts toward treating illness as a personal failing gets both the ethics and the epidemiology wrong.
Two camps that distrust each other profoundly have arrived at the same device from opposite directions, one seeking to hand people power over their own health, the other seeking a better instrument for pricing risk. What makes that convergence consequential is not that either side is lying about its intentions. It is that intentions do not travel with infrastructure. The record of your heartbeat and your sleep is inert about why it was collected, and once it exists at scale, what it does is determined by the rules governing who may see it and what they may do with what they see. That is why the argument worth having is not really about whether wearables are good, a question that probably has no general answer, but about the far less exciting matter of what the law permits to be done with the data. If the protections against health-status pricing hold, this is a story about privacy and hype. If they give way, it becomes a story about a health movement that distrusted institutions and, in good faith, helped build the most detailed personal risk file those institutions have ever had. Which of those futures arrives will be settled in regulatory language, not in any decision anyone makes about what to strap on their wrist.
